Wellness Program for Staff & Spouses

Be FINANCIALLY REWARDED for your steps toward good health!

The District has partnered with Quality Health Solutions (QHS) to offer the Wellness Program to staff and spouses enrolled in the District's medical insurance plan. By promoting healthy living, the District hopes to contain health care costs and provide you with affordable health care benefits. The program goal is to have a vital and healthy population of staff and families and to develop a culture that supports and rewards healthy lifestyle choices!

All Sioux Falls School District staff on the medical plan and their covered spouses are eligible to participate in the Wellness Program. District staff and their covered spouses, if applicable, who complete Step 1 and 2 below will receive the Wellness Rate (discounted medical plan premiums) for the entire 2014-2015 plan year. Those who choose not to complete the Wellness Program will pay more for their 2014-2015 medical plan premiums.

Please note: If you have a covered spouse on your medical plan, both you and your covered spouse must complete the Wellness Program to be eligible for the Wellness Rate in the 2014-2015 plan year.

Step 1: Complete the Quality Health Survey by March 1, 2014*

It’s time to take your annual Quality Health Survey. Even if you participated in the last program year, you must complete the Quality Health Survey again to earn or maintain the Wellness Rate in the 2014-2015 plan year.

Step 2: Submit a Completed Preventive Care Form by March 1, 2014*

SFSD encourages you to use your benefits to stay current with preventive care services. Please schedule a wellness visit with your health care provider and obtain all appropriate health screening tests, including lab work, listed on the enclosed Preventive Care Form. Fax your completed form to QHS at 503-206-3083.

QHS will confirm receipt of your fax if you provide a phone number or email address on the form for this purpose.

Important: You need to have a current annual exam with a date of service between March 16, 2013 and March 1, 2014.

*New employees receive the Wellness Rate. Employees hired on or after November 1, 2013 will receive the Wellness Rate (reduced medical insurance premiums) for the remainder of the 2013-2014 plan year and the 2014-2015 plan year without completing the Quality Health Survey or Preventive Care Form requirements. However, in order to retain the Wellness Rate for the 2014-2015 plan year, these employees must complete the Wellness Program requirements by March 1, 2015.
